JOB SUMMARY
: The Youth Substance Use Liaison serves as a liaison between West County Health Centers, school staff, young people, and their families, with a specific focus on substance use and harm reduction. Responsible for providing youth-directed services through a range of activities, such as case management, support, and education. Will serve as a member of staff at the West Sonoma County school assigned. Responsible to co-lead/manage the Peer Education Program and its volunteers. Will participate in student, family and community engagement activities related to harm reduction, substance use diagnosis and treatment options, and resource navigation in various school, classroom and community settings.
DUTIES:
School Based Health Services:
Foster positive relationship between school and West County Health Centers Collaborate with community organizations and school to meet needs of students and their families Work weekly on assigned school campus (may include after hours or weekend events throughout school year) to foster community and relationships with staff and students Attend school staff meetings at assigned school Provide health coaching, patient navigation and health education for students within the schools Work extensively with school and community agencies and have a working knowledge of how to connect students and their families with internal (WCHC) and external resources Conduct outreach within the schools and the community Conduct daily data entry within AthenaOne (patient health records system) to track patient encounters and progress over time, aiding in program evaluation efforts
Education and Community Engagement:
Implement substance use and harm reduction education curriculum in middle school and high school classrooms. Communicate and collaborate with teachers and administrators to implement education programming. Regularly evaluate and update curriculum materials with other Liaison and Program Manager. Document completely and accurately all presentations and activities for tracking and evaluation purposes. Stay current on adolescent substance use and harm reduction issues. Support access to harm reduction resources in schools and community. Participate in community engagement around substance use and harm reduction. Help with student and community co-design engagement for program design within the school and community settings.
Skills and Experience:
BA or BS in Health Education, Psychology, Social Work, Education or related field desired or like experience Experience providing health education in school settings and/or community health centers. Experience working with at-risk adolescents of all backgrounds in school settings or in agencies providing similar services. Experience with substance use education, navigation or treatment desired but not required. Current certification in first aid and CPR (required within two months of employment) Must have access to reliable transportation to travel between the clinic and school campuses Basic office and equipment safety, knowledge and practice Basic computer skills Ability to work well with a multidisciplinary team and autonomously Ability to problem solve and be self-motivated with attention to detail Must have excellent written and oral skills, strong organizational and analytical skills Must have ability to manage priorities and workflow Must have flexibility and a willingness to work within constantly changing priorities with enthusiasm and positive attitude Expected to have strong interpersonal skills and ability to work well with others •Required to successfully pass background check run by the assigned local school system• Physical Requirements Ability to move freely about the clinic/office and carry light loads (up to 25 lbs.) Prolonged standing and /or sitting required. Ability to interface with electronic office equipment for extended periods of time.
