Social Worker Care Manager (SWCM)

Part-Time 20 Hours per week
Salary:
$22.66 per hour Catholic Charities Diocese of Stockton's Multipurpose Senior Services Program (MSSP) has an opening for a Part-Time Social Worker Care Manager (SWCM) position, 20-hour week. The SWCM provides a wide variety of care management services to a diversified participant population of older adults (60+) living in San Joaquin County with the goal of preventing premature nursing facility placement. By utilizing a comprehensive interdisciplinary team approach, the SWCM, Supervising Care Manager, and Nurse Care Managers design a care plan focusing on the participants needs and resources required to facilitate their living at home independently and safely. The position requires the SWCM to conduct assessments/reassessments for a case load of 25+ participants each month and to timely enter progress notes into a CDA secured client portal. The SWCM will also interact with physicians, medical specialists, service vendors, and other community organizations. Identifies and develops support systems for the participant.
Requirements:
Bachelor's degree in social work, psychology, counseling, rehabilitation, gerontology, sociology, or related field, plus two years of experience working with frail older adults.
