POSITION SUMMARY
The Social Worker provides psychosocial and discharge planning services to all inpatients and select outpatients as identified through the Utilization Review Program. Responsibilities include but are not limited to: Planning, organizing, and implementing social work, discharge planning, and post-acute care services for patients and families through patient and family engagement and based on their goals and preferences. Demonstrates proficiency in navigating relevant community services to reduce and minimize barriers to patients' access and use of community resources. Identifies patients with risk of abuse, neglect or exploitation and intervenes per California mandatory reporting processes. Collaborates with the Resource & Utilization Management staff in assuring appropriate services to the patient in a cost-efficient manner. Coordinates services to promote continuity of services post discharge as appropriate. Demonstrates proficiency in delivering care to assigned age specific patient population (e.g., infants, adolescents, adults, geriatrics, pediatrics) and participates in related continuing education. Must be able to perform jobs in all patient care and operational areas subject to reasonable accommodations, also be able to employ all mandatory safety devices and procedures. Must participate in the Respiratory Protection Program, which may include the wearing of a respirator. This is a non-union position. See Essential Job Functions.

POSITION QUALIFICATIONS
Education:
Graduate of an accredited college or university with a baccalaureate or advanced degree in social work. MSW preferred.
Experience:
1 -3 years in related field or similar situation preferred.
