  • 4:30 PM Occasional weekend shifts required Position Overview As a Social Services Practitioner (MSW), you will provide clinical psychosocial services to patients and families across the lifespan, from neonates to geriatric populations.
You will assess psychosocial needs, coordinate community resources, provide crisis intervention, and support patients and families during challenging life events. Key Responsibilities Conduct psychosocial assessments and develop appropriate care plans. Provide counseling and consultation to patients, families, and healthcare staff. Coordinate referrals to community, social, health, and welfare agencies. Deliver crisis intervention and bereavement counseling services. Participate in interdisciplinary meetings and collaborative care planning. Provide training and support for nursing staff during on-call hours and weekends. Utilize effective problem-solving and decision-making strategies. Perform additional duties as assigned. Qualifications Required Master's Degree in Social Work (MSW) or equivalent combination of education and experience. Minimum of one (1) year of relevant experience in a healthcare, social service, or behavioral health setting. Basic Life Support (BLS) certification. Crisis Prevention Intervention (CPI) certification. CPI training can be provided onsite after hire if needed. Preferred Three (3) years of experience in a social agency, hospital, or healthcare organization. Licensed Clinical Social Worker (LCSW).
