Job Description
Under the general supervision of a program manager, the Maternal, Child, and Adolescent Health (MCAH) Community Health Worker assists professional staff in providing community outreach, education, investigation, case coordination and assistance.

THE MCAH PROGRAM
The MCAH program aims to enhance the health outcomes of reproductive-age women, infants, children, and adolescents. It works to address key public health concerns such as maternal and infant mortality, child development, and adolescent health. The program promotes health equity and strives to provide comprehensive, quality healthcare to underserved populations.THE IDEAL CANDIDATE
The ideal candidate is a self-motivated individual with strong communication in English and Spanish, as well as strong organizational and outreach skills. They are proficient in Microsoft Word, Excel, and PowerPoint, and can design outreach materials such as flyers and presentations. Comfortable engaging with both individuals and groups, they demonstrate confidence in public speaking and the ability to build trust across diverse communities. The ideal candidate is knowledgeable about community resources, MCAH programs, and related medical and social services. They excel in coordinating events, tracking outreach efforts, maintaining accurate documentation, and working collaboratively in team settings.EDUCATIONAL INCENTIVE
Some positions may be eligible for an educational incentive. This incentive may be 2.5%, 3.5%, or 5% for incumbents in eligible positions based on completion of an Associate's, Bachelor's, or Master's degree that is not required for the classification.DEPARTMENT/AGENCY
Health Care Agency - Public Health Community Health Worker is represented by the Service Employees' International Union (SEIU) and is eligible for overtime compensation. The eligible list established from this recruitment may be used to fill current and future Regular (including Temporary and Fixed-term), Intermittent, and Extra Help vacancies for this position only. There is currently one (1) full-time, Regular vacancy within Public Health.TENTATIVE SCHEDULE OPENING DATE

April 29, 2026 at 5:00 PM Duties may include but are not limited to the following: Participates in client intake by assisting in initial client assessments and follow-up, including basic intake interviews and screenings; Assists individuals or families to define problems and needs, and places them in order of priority; Assists in client referrals to community organizations and medical offices; Tracks groups and community outreach interactions, keeping appropriate records; Addresses barriers to care by linking clients to maternal, child, and adolescent health resources and provides care management services; Follows program guidelines and regulations, including program eligibility; Participates in care team meetings and required training; Conducts community outreach by acting as Community Health liaison with community groups and clients and informs them about community services, and participates in focus groups in the community on health and social problems; Conducts house-to-house visits for interviews to gather and provide information about maternal, child, and adolescent health and social service programs; and Performs other related duties as required. These are entrance requirements to the exam process and assure neither continuance in the process nor placement on an eligible list. EDUCATION, TRAINING, andEXPERIENCE
High school diploma, GED or equivalent, plus any combination of education and experience which has led to the acquisition of the required knowledge and abilities.NECESSARY SPECIAL REQUIREMENTS
Must be bilingual in English and Spanish Experience in public speaking and creating presentations Ability to work in a Microsoft Windows environment Intermediate experience using Microsoft Word, PowerPoint, and Excel Must possess or obtain a valid California driver's license by date of hire and maintain it thereafter DESIRED Experience collaborating and working with people from diverse backgrounds Experience working in a Public Health environment KNOWLEDGE, SKILLS andABILITIES
Some to thorough knowledge of: community service organizations that provide services to the MCAH program's target population; Maternal, Child, and Adolescent Health program policies and procedures; group and family dynamics; and, general knowledge of Medical guidelines.Skilled in:
Computer applications such as a Windows environment utilizing Microsoft Word, Excel and PowerPoint; public speaking and creating presentations; and, developing trust and effective working relationships with a diverse client population.Ability to:
Effectively communicate with individuals and groups from diverse cultural, socio-economic backgrounds; maintain case management tracking notes and reports in computer system; monitor client compliance with the Maternal, Child, and Adolescent Health program; lead and/or work effectively in a team environment; drive to client homes to conduct interviews; and, speak English and Spanish.FINAL FILING DATE

%: An un-proctored, online, job-related written examination will be administered to test candidates' knowledge of language usage, interviews/confidentiality, medical knowledge and terminology, interpersonal relations, and filing. Candidates must earn a score of seventy percent (70%) or higher to qualify for placement on the eligible list.
