Job Description

We are looking for a BSW or MSW to provide social work services to our participants.
AREAS OF SOCIAL WORK RESPONSIBILITIES
1. Processes all inquiries in a timely manner and maintains tracking documentation. Meets with family members, or referring agency, and determines appropriate program. 2. Provides in-home assessments for all ADHC participants. Completes social service assessments on all participants. 3. Provides discharge planning for all participants. 4. Assesses and makes recommendations for financial assistance needs of participants. 5. Assesses and determine the eligibility of participant for the child and adult care food program. 6. Complete child and adult food program participation eligibility application for each eligible participant. 7. Provides full case coordination for ADHC participants and comprehensive information and referral resource file for the center's primary and secondary service areas along with pertinent referrals outside of service area. 8. Provides signed and dated documentation for all services performed on the day the service is provided. 9. Completes signed and dated quarterly progress notes, and six-month reassessments on all participants. 10. Provide functional assistance in the areas of assisting participants in any area of their personal care needs, program needs. 11. Provides group sessions in the program. 12. Provides participant and family counseling as needed. 13. Facilitates, or co-facilitates, caregiver support groups as assigned. 14. Maintains a working knowledge of facility service areas including transportation, community characteristics and geography. 15. Active participant in the ADHC Multi-Disciplinary Team 16. Active participant in all staff and in-service meetings. Requirements may require outside training. Provide training for in-services as assigned. 17. Works as an integral participant of a management team concept. Maintains effective relationships with staff geared to the building and maintenance of a cohesive team. 18. Consults with and advises staff members as to the relationship of social, emotional and ethnic factors to health and medical care, and as to the availability of social services in the community. 19. Maintains all agency, licensing, contracts and administrative requirements for documentation as required for client services and administrative needs including raw data collections and tabulations and reporting requirements. 20. Assist in developing and providing educational and training programs for caregivers and professionals. Assisting in providing fact sheets and other information materials. 21. Acts as an advocate for elderly concerns and provides referrals to persons inappropriate for the adult day health care center. 22. Participates as a member of assigned community committees as a representative of New Sunrise Adult Day Health Care Center. 23. Maintains a networking liaison with other organizations in the eldercare field. Represents the agency in contact with human service, health care, and community organization groups and individuals. 24. Primary person responsible for providing marketing and public relations efforts for the facility. Plans will be established with administration including goals and tracking. 25. Maintain up-to-date procedures manual and master calendar for position. 26. Assist with agency fund raising efforts. 26.
