JOB DETAILS POSITION SUMMARY
- LCSW Social Worker
POSITION DUTIES
- Provide clinical assessment, crisis intervention, care coordination, and discharge planning to help patients and families navigate medical, emotional, and social needs during hospitalization.
MINIMUM REQUIRED QUALIFICATIONS
- CO
LCSW BLS 2
years social work experience in an acute hospital setting Acute hospital experience in specialty during the past year with no more than a 90-day breakPREFERRED QUALIFICATIONS
- ED experience Palliative Care Knowledge and GOC Conversation Behavioral Health experience with suicide risk and substance use assessments Discharge planning experience
