Connecticut Renaissance is a CARF-accredited organization known for providing comprehensive behavioral health services for adolescents and adults living with addiction and/or mental health disorders. A leader in using evidence- based practices, we offer quality substance abuse and mental health treatment to a diverse population across 12 Connecticut locations. We also provide case management, housing, and life-skills training for individuals transitioning from incarceration back into the community.
Position Summary:
The Family Advocate in the LYNC Program (Linking Youth to Natural Communities) serves as the primary point of contact for enrolled youth and their families throughout the duration of program participation. This role is responsible for delivering culturally responsive, gender-responsive, trauma-informed, and strength-based services designed to support youth development, family engagement, and connection to natural community supports. The Family Advocate will work collaboratively with youth, families, and internal/external service providers to ensure coordinated care and successful program outcomes.
Schedule:
Monday through Friday Three (4) days: 10:00 a.m. - 6:00 p.m. Two (1) days: 11:00 a.m. - 7:00 p.m. Rotating Saturday (every 6-8 weeks): 8:30 a.m. - 12:00 p.m. Flexibility is required to meet the needs of youth and families served by the program. Connecticut Renaissance Offers a
Competitive Salary Essential Duties and Responsibilities:
Serves as point of contact for youth and families, service delivery in program, and connections to the community. Work with youth and families to mitigate barriers and increase likelihood of youth and family success. Engages and connects youth and families to services in LYNC and in the community. Maintains compliance in documentation with agency and program standards. Participates in staff meetings, supervision, and completes trainings timely. Participates in all required Quality Assurance activities. Assigned to the youth and family through all three phases of the LYNC program. Provides structured case management, works on goal development, addresses barriers, develops and builds skill acquisition. Works closely with the LYNC Service Specialist to connect youth and families to community services and/or group interventions.
Required Knowledge, Skills, Abilities and Other Characteristics:
Knowledge Of:
Case Management and community resources. Electronic Healthcare Record systems. General office software, particularly Microsoft Office Suite.
Ability To:
Communicate effectively both verbally and in writing. Manage priorities effectively to meet deadlines and demonstrate multi-tasking skills. Assimilate new information and technology.
Education and Experience:
Bachelors in Criminal Justice, Sociology, or Human Services or a minimum of three years experience working with the justice/system/agency involved youth. Strong case management skills. Effective engagement skills with motivational interviewing skills preferred. Strong community connections and knowledge base of resources. Demonstrated understanding of risks-needs-responsivity principles and risk reduction initiatives.
Other:
Must be dependable and reliable; detailed oriented; and able to interface easily with clients, staff, and referral sources. Must be able to carry up to 20 pounds Must have a valid Driver's License.
