Assessment & Care Planning:
Complete comprehensive psychosocial assessments upon admission and develop personalized care plans, including 90-day updates.
Advocacy:
Protect residents' rights, address complaints or reports of abuse/neglect, and act as a liaison between residents, families, and staff.
Discharge & Care Coordination:
Facilitate safe discharge planning, coordinate with community resources (e.g., home health, hospice), and work with the interdisciplinary team.
Documentation:
Maintain accurate, timely, and confidential records of social services, progress notes, and assessments in compliance with state/federal regulations.
Experience:
At least one year of supervised social work experience in a healthcare or long-term care setting.
Knowledge:
Strong understanding of Medicare/Medicaid regulations, residents' rights, and, state/federal, 6 rules.
Skills:
Excellent communication, crisis intervention, conflict resolution, and empathy.
