Job Description

Job Title:
Case Coordinator -
Social Worker Overview:
We're seeking a skilled Social Worker to coordinate inpatient discharge planning, ensure appropriate and timely care, and support utilization management. You'll work closely with patients, families, physicians, nursing, payers and community providers to create smooth transitions to post-hospital care and resolve complex cases.
Key Responsibilities:
Assess patient care plans and progress using medical records and team input; ensure alignment with payer/industry criteria. Identify and resolve delays in tests, procedures or specialty evaluations; coordinate with internal and external providers to expedite care.
Manage complex discharge planning:
recommend post-acute options, develop plans with physicians, nurses, patients and families, and arrange community services. Lead or participate in multidisciplinary case conferences to develop tailored transition plans. Document case coordination and utilization activities in Epic (CM flow sheets, AUTTH/CERT). Accept and document verbal discharge orders; communicate plans to care team. Act as liaison to payers and external providers for authorization, placement and post-discharge services. Screen and refer patients for protective services, conservatorship, elder services, newborn home care resources, and Physician Advisor review for medical necessity when needed. Collect and analyze avoidable day and utilization data; identify trends and opportunities for improvement. Provide staff education on case coordination best practices and participate on hospital committees.
Qualifications:
Master's degree in Social Work or related field (sociology, gerontology, rehabilitation counseling, psychology). Licensed Social Worker in the District of Columbia. 2-5 years healthcare social work experience, including at least 1 year in discharge planning and 1 year in inpatient utilization management/case coordination using InterQual, M&R or similar criteria. Current CPR certification. Proficient with electronic health records (Epic preferred) and community resource networks. Strong communication, negotiation and interdisciplinary collaboration skills. Ability to work flexible hours and move throughout the hospital.

Salary Range:
Minimum 40.35/hour - Maximum 66.54/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
