Virginia Williams Family Resource Center, 64 New York Ave NE, Washington, DC 20002 Job type: Full-time • Contract (up to 3 years) • Fully onsite About the role Changing Technologies Inc is hiring a Case Manager to support the DC Department of Human Services at the Virginia Williams Family Resource Center — the District's central intake center for families experiencing homelessness and housing instability. The Case Manager guides families through the full case lifecycle, from intake and assessment to coordinated services, advocacy, and discharge planning. Responsibilities Admit new families by reviewing records and applications, conducting orientations and intake interviews, and determining needs for referrals and interventions. Help families establish schedules and routines, coordinate the services they receive, and arrange resources — including transportation. Monitor each case by verifying attendance, evaluating treatments and responses, advocating for needed services and entitlements, securing additional resources, intervening in crises, and supporting clients and families. Maintain accurate client records by keeping case notes and logging events and progress. Lead weekly interdisciplinary meetings and evaluations, communicating each family's progress, results, and obstacles to the therapeutic team and family members. Coordinate discharge and post-discharge planning, orient and train family members, and connect them to community resources. Prepare reports by collecting, analyzing, and summarizing treatment data and trends, compiling statistics, and completing grant and subsidy applications. Maintain professional knowledge through continuing education, publications, and professional networks, and take ownership of new and evolving requests. Qualifications Bachelor's degree in social work, psychology, counseling, human services, or a related field is preferred. Minimum 2 years of case management or direct social-services experience, ideally with families, housing, or homeless services. Strong skills in client assessment, crisis intervention, documentation, and resource coordination. Comfortable working with electronic case-management systems. Knowledge of DC social-services and entitlement programs is a plus. Compensation & schedule Pay:
