JOB DETAILS POSITION SUMMARY
- LMSW Social Worker
POSITION DUTIES
- Provide clinical assessment, crisis intervention, care coordination, and discharge planning to help patients and families navigate medical, emotional, and social needs during hospitalization.
MINIMUM REQUIRED QUALIFICATIONS
- FL LMSW or
LCSW BLS 2
years social work experience in an acute hospital setting Acute care adult hospital case management experience Acute hospital experience during the past year with no more than a 90-day break Knowledge of Hospital DC planning/execution and knowledge of MDR or IDT MIDAS/Meditech, PASRR, and Navihealth experienceLENGTH OF ASSIGNMENT

- 8:00AM
- 4:30PM and every third weekend. SYSTEMS
- MIDAS/Meditech, Nate, Navihealth, Care Team Assignment
