Baptist Health is seeking a compassionate and motivated Social Worker II to join the team at Wolfson Children's Hospital. This Full-Time evenings/weekends position plays a vital role in supporting pediatric patients and their families by addressing psychosocial needs and ensuring access to essential services and resources. Key Responsibilities Assess patients' psychosocial needs to support emotional well-being and optimal functioning. Provide discharge planning , including coordination of home health services and durable medical equipment (DME). Assist with patient placement and transfers , ensuring smooth transitions of care. Offer referrals to community resources to support ongoing care and family needs.
