Our centers of excellence include Baptist MD Anderson Cancer Center, Baptist Heart Hospital, Baptist Neurological Institute and Wolfson Children's Hospital. Baptist Health is seeking a dedicated and compassionate Master's Level Social Worker (Social Worker II) to join our team. This is a Full-Time, Day Shift opportunity working at our Baptist South campus. In this role, you will be a key member of an interdisciplinary care team , working to improve overall well-being by addressing the psychosocial needs that impact patients' lives. The focus is on reducing barriers to care and easing the burden of medical conditions through emotional support, education, and resource connection.
Social Worker II, Job Responsibilities:
Assess psychosocial needs and deliver services to enhance patient functioning and emotional well-being Coordinate care to ensure continued access to support and services Provide psychoeducation to patients and families Connect individuals with appropriate community resources and referrals
Experience Requirements:
3-5 Years Discharge Planning Experience 3-5
Years Case Management Experience Education & Credential Requirements:
