Monday - Thursday 9:00am to 5:00pm and Friday 9:00am to 3:00pm.

Job Summary:
This position provides community-based case management services to adults, children, adolescents, and families to support recovery, resilience, and optimal functioning. The role focuses on linking clients to resources, coordinating care, and promoting independence through a holistic, strengths-based approach tailored to individual needs.
Key Responsibilities:
Conduct comprehensive assessments and evaluations of client needs across the lifespan (adults, youth, and families). Develop, implement, and regularly review individualized service plans in collaboration with clients and the treatment team. Provide community-based case management services, including home visits, to support engagement, wellness, and safety. Coordinate and monitor service delivery to ensure continuity of care across providers and settings. Link clients to community resources and support services (e.g., healthcare, housing, financial assistance, education, vocational services). Advocate for clients to ensure access to appropriate services and supports. Assist clients with skill development, including independent living, transitional skills, and vocational readiness. Facilitate family involvement and promote natural support systems. Provide crisis intervention and support as needed. Support discharge planning and coordination of aftercare services. Collaborate with internal teams and external agencies to ensure effective service coordination. Maintain accurate and timely documentation, including client records, case notes, and required administrative forms. Provide transportation for clients when necessary. Function as an active member of a multidisciplinary team.
Qualifications:
Bachelor's degree from a regionally accredited college or university in Psychology, Social Work, Behavioral Science, or a related field (Required) Experience working with adults, children, and/or families with mental health and/or substance use disorders (Required/Preferred depending on leveling) Knowledge of behavioral health treatment concepts, community resources, and applicable regulations (Required) Obtain Certified Case Management credential within 12 months of hire (Required) Valid Florida Driver's License with a clean driving record (Required) Ability to work a flexible schedule based on client needs (Required) Strong organizational, time management, and problem-solving skills (Required) Effective communication skills with diverse populations, including children, adults, and families (Required) Ability to exercise sound judgment in crisis or complex situations (Required) Accurate documentation and record-keeping skills (Required) Bilingual skills (Preferred) Microsoft Office 365 skills (Including Teams/Video Conferencing) and Electronic Medical Records experience = Strongly Preferred Staying compliant on all training and immunizations. Drug Screen and Local Criminal Check Physical Requirements:
This position requires a mix of physical activities throughout the workday. Duties involve periods of standing, sitting, and walking, often across various areas of the site. The role also includes tasks that may require bending, reaching, and other physical movements. Candidates must be able to occasionally lift and carry items weighing up to 50 pounds. A reasonable level of physical stamina and mobility is necessary to successfully perform the essential functions of this role.
