Day Shift Description:
The Social worker will work in collaboration with case managers and other healthcare professionals to enhance cost effective use of resources by identifying and initiating complex discharge planning to accomplish the timely transition of patients to post-hospital settings. The social worker must have in-depth knowledge of system and community resources and referral agencies. They are responsible for communicating with external agencies to provide referral information for a smooth transition form hospital to the post-hospital setting. This includes communicating with other health care agencies and payer sources. The social worker will also provide assistance to patients, families and others with a wide variety of needs to include, but not limited to, discharge planning, financial concerns and/or emotional problems related to the current acute illness. Provides supportive counseling to patients regarding adjustments to illness as well as other psychosocial needs as indicated.
Position Requirements:
A Certification/Registration:
Current MSW degree required B.
Education:
Must be a graduate of a Master of Social Worker Program.
C:
Experience:
Must have a minimum of 3 years experience in social work, including a minimum of 2 years in hospital discharge planning D.
