Job Description

Social Worker - Skilled Nursing Facility (SNF) Position Summary The Social Worker in a Skilled Nursing Facility (SNF) is responsible for supporting residents and their families with psychosocial, emotional, and discharge planning needs throughout their stay. This role works collaboratively with the interdisciplinary care team to promote resident well-being, ensure regulatory compliance, and facilitate safe transitions of care. Key Responsibilities Conduct psychosocial assessments for residents upon admission and as needed. Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team. Provide emotional support, counseling, and crisis intervention to residents and families. Coordinate discharge planning, including referrals to community resources, home health services, hospice, or other post-acute care services. Assist residents and families with advance directives, guardianship concerns, and adjustment to illness or long-term placement. Advocate for resident rights and ensure compliance with federal, state, and facility regulations. Participate in care conferences and interdisciplinary meetings. Maintain accurate and timely documentation in the electronic medical record (EMR). Support residents with behavioral health concerns, grief, loss, and end-of-life issues. Educate residents and families regarding available services, insurance coverage, and community resources. Investigate and report concerns related to abuse, neglect, or exploitation in accordance with facility policies and regulations. Assist with admissions, transfers, and resident transitions as needed. QualificationsRequired Bachelor's degree in Social Work (BSW) or related human services field. Current state licensure/certification as required. Minimum of 1 year of experience in healthcare, long-term care, rehabilitation, or skilled nursing preferred. Knowledge of Medicare, Medicaid, discharge planning, and long-term care regulations. Strong communication, organizational, and interpersonal skills. Ability to work effectively within an interdisciplinary team.
