Job Description

Key Responsibilities Provide a wide range of psychosocial services to patients and their families Conduct ongoing assessments of patient and family psychosocial needs Evaluate and support pre-bereavement and anticipatory grief needs Develop and implement individualized care plans to address emotional, social, and practical challenges Offer supportive counseling, problem-solving assistance, and community resource referrals Continuously re-evaluate patient and family needs and adjust care plans accordingly Participate in interdisciplinary team meetings to discuss patient care and identify potential risks Take part in on-call rotation as assigned Assist patients and families with planning for funeral arrangements, financial matters, legal considerations, and healthcare decisions Provide education and consultation to care team members regarding psychosocial aspects of care Perform additional duties as assigned to support team and organizational goals Minimum Qualifications Master's Degree in Social Work (MSW) Active LMSW licensure in the state of employment Clinical experience or training related to death, dying, and bereavement Knowledge of relevant state and federal regulations Strong communication, interpersonal, and organizational skills Ability to work independently and collaboratively within a team Valid driver's license and auto insurance Equivalent combinations of education and experience may be considered Benefits Paid Time Off (PTO), Sick Leave, and Holiday Pay Comprehensive Health Benefits (Medical, Dental, Vision) for full-time employees 401(k) Retirement Plan Gas Allowance and Car Maintenance Stipend Employee Assistance Program (EAP) Tuition Reimbursement (for eligible employees) Opportunities for Internal Career Growth and Advancement Ongoing Training and Professional Development
