Job Description

Job Summary We are seeking a dedicated and compassionate Social Worker for a travel contract to provide vital mental health and social support services across diverse healthcare settings. In this dynamic role, you will assess patient needs, develop personalized care plans, and deliver a wide range of counseling and therapeutic interventions. Your expertise will help improve patient outcomes, promote well-being, and support families through challenging times. This position offers the opportunity to work in various environments, including hospitals, outpatient clinics, inpatient units, and community-based settings, all while making a meaningful difference in people's lives. Responsibilities Conduct comprehensive patient assessments to identify mental health needs, behavioral challenges, and social determinants affecting well-being. Develop and implement individualized treatment plans incorporating clinical counseling techniques such as cognitive behavioral therapy (CBT), motivational interviewing, and group therapy. Provide crisis intervention and behavioral health support to patients experiencing acute mental health episodes or trauma. Collaborate with multidisciplinary teams to coordinate care, ensure compliance with HIPAA regulations, and maintain accurate medical documentation using Electronic Medical Record (EMR) systems. Facilitate family meetings, child & family counseling sessions, and specialized therapies for individuals with developmental disabilities or autism spectrum disorder. Manage casework responsibilities including intake procedures, discharge planning, and ongoing case management for diverse populations such as seniors in assisted living or patients with dementia or PTSD. Collect data for clinical research projects related to behavioral health interventions and patient outcomes while adhering to confidentiality standards. Experience Proven experience working in hospital settings or inpatient units with a strong understanding of medical terminology and documentation practices. Demonstrated expertise in child & family counseling, behavioral therapy, addiction counseling, or hospice care. Hands-on experience with home & community care programs or post-acute care environments is highly desirable. Familiarity with EMR systems, CPT coding (Current Procedural Terminology), ICD coding (International Classification of Diseases), ICD-9/10 classifications, and medical records management. Background working with individuals with disabilities—including developmental delays, dementia, Alzheimer's disease—or in special education contexts is preferred. Knowledge of crisis management strategies and experience providing mental health counseling across outpatient or outpatient clinic settings. Ability to work effectively with diverse populations including pediatrics, geriatrics, or individuals requiring specialized care such as PTSD or Alzheimer's support.
