Job Description

Cottages on Mountain Creek , Atlanta GA Clinical Therapist/ Case Manager & Mental Health Group Facilitator Facilitate mental health group discussions, case manage 24/7 clients and reintegrate clients into community after relapse.
  • Develop relationships and build rapport with a caseload of 9-12 weekly clients using active listening and client centered counseling techniques
  • Collaborate with treatment team to assess, diagnose, and develop an effective treatment plan for the client
  • Monitor client progress toward treatment goals to ensure appropriate clinical interventions and adjustments to overall plan
  • Determine nature of client's situation by interviewing client; assessing medical, psychological, emotional, and social information
  • making on-site visits
  • Establish course of action by exploring options; setting goals with client
  • Ensure accurate case notes and confidential client files
  • Communicate with family monthly as well as as-needed regarding clients progress
  • Obtain assistance for client by referring him/her to community resources; arranging for appointments; establishing rapport with other agencies
  • Provide weekly case management sessions for adults using person-centered, CBT, and psycho-dynamic approaches
  • Develop and lead successful new weekly process group for adults managing psychosis (3 hrs)
  • Document client progress in IOP sessions and interaction (BIRP NOTES)
  • Foster client's action or adjustment by interpreting attitudes and patterns of behavior; explaining and pointing out new options
  • Maintain record of case by documenting client's situation and client's own actions.
  • Monitor planned actions by periodic follow-up
  • Implement monthly treatment plan meeting with family and client
  • Weekly case management meeting with client
  • Maintain operations by following policies and procedures; participating in quality reviews; reporting needed changes
  • Comply with federal, state, and local legal requirements by studying existing and new legislation; enforcing adherence to requirements; advising management on needed actions
  • Maintain client confidence and protects operations by keeping information confidential
  • Conducted extensive research and developed policies and procedure manual as well as patient care best practices
  • Complete monthly Quality Measures documentation and progress
  • Complete monthly schedule and adjust and maintain communication with adjunct therapists
  • Complete house checks weekly with focus on independent living skills
  • Weekly supervision meetings with clinical supervisor
