Gwinnett County Public Schools Primary Responsibilities:
Responsible for promoting and enhancing the overall academic mission by alleviating learning barriers and assessing the needs of students, families, and schools and implementing direct case services, crisis intervention services, and advocacy support within the assigned cluster(s) or specific location. Student Centered Support and Interventions. Provide direct case services to address academic, social, and emotional concerns.
Direct case services include student check-ins, small groups, conflict resolution, crisis intervention, home visits, and community service coordination.
Conduct home visits after consultation with administration and /or upon school social worker assessment to establish effective communication between the parent/guardian and school.
Provide crisis intervention support to students, families, and local schools for situations affecting attendance, social /emotional health, and school environment.
Provide student centered services to assist with adjustment, attendance, academic achievement, mental health issues, and social emotional learning. Coordination and Collaboration. Serve as the liaison between families and schools concerning compliance with the state compulsory attendance law.
Collaborate with local schools to address student social emotional issues.
Coordinate student/family utilization of community resources and facilitate access of applicable services through school social work delivery model.
Consult with local schools and the Department of Family and Children Services concerning all forms of suspected child abuse and neglect.
Serve as a liaison between the district and community services agencies. Planning and Program Management. Assist in the planning, implementation, and evaluation of programs identified to promote academic achievement.
Conduct student, family, and school needs assessment.
Develop long-term and short-term intervention plans consistent with needs assessment.
Advocate for appropriate services on behalf of students families and schools.
Actively participate in appropriate professional development and monthly staff meetings to enhance knowledge and skills. Data Collection, Documentation, and Reporting. Complete and submit monthly reports in accordance with departmental standards to the Director of Health and Social Services.
Maintain documentation of provided student services, case files, and all other student information in accordance with GCPS policy and procedure.
