Job Description

Duties:
  • Provide case management services to children in need of childcare
  • Participate in direct care of foster children in Choices for Life of Georgia foster homes.
  • Participation in the admissions process and placement of the child in the foster home, develop an individualized service plan (ISP and assist wit hits implementation that includes the child's medical, dental, educational and therapeutic services.
  • Be the point of contact for the foster parent, DFCS, Court Systems, Guardian ad litem, Court Appointed Special Advocates, school systems, medical/dental providers, therapist/community support individuals and other professionals involved in working with the child.
  • Coordinate with parents, guardians, and other professionals involved in the child's care
  • Monitor and evaluate the progress of each child in meeting their goals
  • Advocate for the best interests of the child and ensure their safety and well-being
  • Collaborate with community resources to provide additional support and services as needed
  • Maintain accurate and up-to-date documentation of all case management activities
Experience:
  • Bachelor's degree in a Human Service field and two (2) years direct service experience working with children and families, or a Master's degree in a Human Service field and one (1) year of direct services.
  • Preference given to individuals who have successfully completed State of Georgia Foundation training, IMPACT certified trainer and possess a valid license in counseling or social work.
  • Previous experience working with children in a childcare setting
  • Knowledge of child development principles and best practices in childcare
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
