Find a fulfilling career and make a difference with UnityPoint Health. Responsibilities Patient Care & Support Conducts psychosocial assessments to identify needs, strengths, and barriers to care. Provides crisis intervention, counseling, and advance care planning support. Connects patients and families with community resources, services, and financial supports. Assists with care transitions and completes required screenings (e.g., PASRR). Advocacy & Education Advocates for patient rights, safety, and autonomy, including reporting concerns as required. Educates patients, families, and care teams on psychosocial aspects of care and available resources. Supports culturally competent care, including coordination of interpreter services. Care Coordination Collaborates with the interdisciplinary team to develop and support patient-centered care plans. Facilitates team and family meetings to promote communication and continuity of care. Partners with internal and community providers to ensure seamless care across settings.

Qualifications Education:

Bachelor's degree in social work from an accredited school of social work

Experience:

Prefer one year clinical experience in healthcare License(s)/Certification(s): Current licensure in good standing to practice in the state where services are provided (BSW) Valid Mandatory Reporter course completion by state requirement Completion of First Steps (ACP) and

IPOST/POLST

training LMSW or LISW preferred.
