Community-Based Rehabilitation Specialist (CBRS) /
Case Manager Job Type:
Full-time Pay:
$23.00-$27.00 per billable hour (based on experience) Position Summary Our agency is seeking an experienced and compassionate Community-Based Rehabilitation Specialist (CBRS) / Case Manager to provide case management and community-based rehabilitative services within our mental health agency. This role supports children and adults with emotional, behavioral, and psychological challenges by promoting recovery, independence, and successful community integration. CBRS staff provide individualized, goal-oriented support in community and home settings, helping participants develop skills, build new roles, and access needed psychiatric, medical, social, residential, financial, and other supportive services. Applicants with a license‑able degree are required to be licensed . Previous experience working with the mentally ill population is required. Key Responsibilities Provide individualized CBRS and case management services in the community and participants' homes Utilize evidence-based approaches and recovery-oriented interventions Support skill development related to daily living, social functioning, and community integration Assist individuals in accessing psychiatric, medical, housing, financial, food, and other community resources Respond appropriately to crises using de‑escalation and intervention techniques Maintain accurate documentation using Microsoft Word and Excel Collaborate effectively with a multidisciplinary treatment team Qualifications Must be 18 years or older Bachelor's degree in behavioral science, health, or a related human services field If degree is license‑able, applicant must hold appropriate licensure Previous experience working with individuals with mental illness ( required ) Ability to pass a background check Proficiency with Microsoft and computer skills.
