Full-time Orchard View Post Acute is seeking a compassionate, licensed Social Worker to support residents and families through every stage of post-acute care. This role is essential in helping ensure safe discharges, coordinating community resources, and providing emotional support in a fast-paced skilled nursing environment. Key Responsibilities Complete psychosocial assessments and develop care plans in collaboration with the interdisciplinary team Provide counseling, crisis intervention, and emotional support to residents and families Coordinate discharge planning, including placement, resources, and follow-up needs Assist with resident advocacy, care conferences, and family communication Support compliance with federal/state regulations and facility policies Maintain accurate, timely documentation in accordance with standards and guidelines Qualifications Active Social Work license in the state of Idaho Experience in skilled nursing, post acute, or long-term care preferred Strong knowledge of discharge planning and community resources Excellent communication, organization, and documentation skills Ability to work collaboratively with clinical and administrative teams Why Orchard View Post Acute Competitive hourly pay based on experience Supportive leadership and team environment Meaningful work with residents and families Apply today to join Orchard View Post Acute and make a difference in the lives of the people we serve!
