Job Description

Community Action Partnership is seeking caring people who want to make a difference in their neighborhood, community, and state! Community Action Partnership is dedicated to building relationships that inspire and equip people to end poverty. We need your help! Are you ready to do something bigger? Supportive Services Coordinator Our team works hard to connect local unhoused with needed resources. The Supportive Services Coordinator will work with the unhoused, assess the need, case manage, and help to connect with services. To be successful in this role, the candidate will need to be adaptable, flexible and compassionate.
Job Responsibilities:
(include but are not limited to) Learn and understand program guidelines and available resources Understand and practice the Continuum of Care process Report organizational outcomes and activities Develop and maintain strong relationships with partner agencies Contact Human Resources for a full job description.
We are looking for:

Hours:
Full time/ Monday to Thursday (32 hrs. a week)
Wage:
$ 24 - $26 (DOE)

Education:
Prefer bachelor's degree in social work, psychology, or other related human services fields. High school diploma minimum requirement. 3-5 years of experience in case management, human services and/or social services
Requirements:
Must pass a background check, and hold a valid driver's license
Pay:
$24.00 - $26.00 per hour
