8am•4pm Monday through Friday with weekend rotation coverage required. Pay Range $30.70•$46.05
Job Description Major Responsibilities:
Provision of direct care to clients in the outpatient program. 1)Evaluates new clients for appropriate treatment modality. Conducts psychodiagnostic assessment (i.e. diagnostic evaluation) according to established clinical protocol, formulates case conceptualization (ie. diagnostic impression of presenting problem), establishes psychiatric diagnosis according to DSM criteria. 2)Conducts individual, marital, family, and group psychotherapy utilizing therapeutic skills and following acceptable clinical guidelines for specific clinical disorders. 3)Formulates goal-oriented treatment plan with expected outcomes in collaboration with the patient and family or group where appropriate, and monitors the progress and outcomes of all the above in accordance with the Illinois Mental Health Code and Good Samaritan Hospital Poicy and Procedures. 4)Educates patient and family (where appropriate) regarding the nature of the patient's illness, it's expected course, treatment of illness and alternative treatments. 5)Assess patient's and the patient family's level of understanding and motivation and level of compliance to engage in treatment, including their expected roles in the treatment process and makes appropriate changes in treatment plan as needed. 6)Develops aftercare plan as part of treatment plan, incorporating community-based resources as appropriate. 7)Productivity is 75% of billed hours. Professional Development, Demeanor, and Conduct 1)Provides clinical services in a professional and customer friendly manner. 2)Conducts self in a professional and courteous manner with concern and respect for both internal and external customer needs. 3)All documentation of clinical services in the patient medical record and maintenance of the medical records are completed according to departmental procedures, hospital policies, and Standards of Practice. 4)Appropriate documentation includes verification of pre-certification of patients and completion of admission documents. The Individual Treatment Plan is completed in a timely manner, documentation of subsequent visits and discharge summary is to be completed in a thorough and timely fashion in accordance with policies set by the department. 5)Attends and participates in all appropriate patient staffings to facilitate multidisciplinary treatment planning and care of patients and families. 6)Participates actively in clinical supervision as deemed appropriate by the Coordinator of Clinical Programs and/or the Outpatient Manager. Program Specific Activities 1)Attends Departmental staff meetings in addition to staff meetings for the outpatient team. 2)Provides input into the development and revision of polcies and procedures. 3)Participates in quality improvement initiatives as indicated.
Licensure:
LSW/LPC OR LCSW/LCPC OR
Psychologist Clinical Education/Experience Required:
Masters degree from an accredited program in Social Work. 2-3 years experience • 8 hours of stroke education/year (if covering stroke units)
Knowledge, Skills & Abilities Required:

Illinois CPR Certification Physical Requirements and Working Conditions:
Additional training/experience in family/group therapy. Resiliency and flexibility to deal with a changing environment. Demonstrated problem solving ability. Ability to work under stressful conditions and in difficult situations. May be exposed to hazardous materials and life threatening diseases. Knowledge of Universal Precautions is a must. Customer focused. Transporting patient is required. May at time come in contact with violent patients, therefore knowledge of techniques to manage violent patients is a must. May be involved in restraining patients. Ability to flex hours in order to meet patient care needs. Ability to handle crisis situations. If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
