Post-Secondary Transition Case Manager/Facilitator Required (1 of the following):
LCPC LCSW PEL
#181 (formerly Type 73) LSW or LPC with a MSW or MA/MS in Counseling Job Details Full-time salary position with benefits (BC/BS health/dental/life insurance, 401K/Match opportunities, Aflac, PTO, tuition reimbursement) Monday - Friday 8:00AM-3:30PM Academic calendar plus summer school session The Post-Secondary Transition Case Manager supports students in the Transition Program by focusing on post-secondary outcomes and student support services. This role serves as the primary case manager/counselor for transition students and works collaboratively with instructional (LBS1) and operational staff (currently the operations role falls to people like Tracy and Lakita) to coordinate services that promote student success in adulthood.
Primary areas of focus include:
Vocational exploration and career development Independent living and daily living skills Social-emotional and mental health support Community access and post-secondary planning Coordination of transition-related services and resources Due to the smaller caseloads within the Transition Program, this role extends beyond a traditional counseling position and includes active involvement in student programming, family collaboration, community partnerships, and individualized transition support. The Post-Secondary Transition Specialist plays a hands-on role in helping students generalize skills across school, community, and vocational settings.
