Job Summary:
We are seeking a dedicated PRN Case Worker to provide exceptional case management services at multiple Deaconess Illinois facilities. The Case Worker will assist patients and their families in navigating healthcare challenges, ensuring that their social, emotional, and practical needs are addressed with compassion and professionalism.
Key Responsibilities:
Conduct comprehensive assessments of patients and families to identify needs and risks, integrating physical, emotional, social, cultural, and spiritual factors. Collaborate with multidisciplinary teams to coordinate care and ensure patient-centered outcomes. Develop and implement discharge plans, including nursing home, rehabilitation, long-term acute care, and community resource referrals. Facilitate transfers to acute care facilities and psychiatric hospitals. Provide accurate information to patients and families regarding available services and support informed decision-making. Offer on-call services for after-hours discharge planning and case management support. Maintain patient privacy and confidentiality in accordance with regulatory requirements. Conduct pre

Monitor length of stay, resource utilization, and discharge timeliness to improve patient flow. Identify patients at risk for readmission and implement effective transition plans. Report child or adult abuse to appropriate authorities as required. Serve as a resource "gatekeeper" to allocate support services efficiently. Ensure documentation is timely, accurate, clinically relevant, and supports team care. Participate in department, care center, and hospital committees or improvement projects as needed.
Education:
Master's Degree in Social Work (MSW)
  • Required Experience:
    Minimum one year of relevant experience in case management within a healthcare setting
  • Preferred Licenses/Certifications:
    State of Illinois License of Social Work (LSW) or License of Clinical Social Work (LCSW)
Required Knowledge, Skills, and Abilities:
Strong analytical and problem-solving skills. Ability to interpret scientific, technical, and legal documentation. Ability to respond to inquiries from patients, families, and regulatory agencies professionally. Proficiency in basic arithmetic and practical problem-solving for patient care scenarios. Why Join Us?
