Any HPMHC office location (Hays, Norton, Phillipsburg, Colby, Goodland, Osborne)
Pay:
$22.36 per hour | $500 SIGN-ON BONUS!

Responsibilities Include:
Facilitating Wraparound planning meetings with families and support teams Helping families navigate the
SED/HCBS
Waiver and PRTF Diversion Grant programs Coordinating services and community resources to support treatment goals Developing individualized plans that focus on strengths, needs, and long-term success Collaborating with families, educators, providers, and community partners Monitoring progress and adjusting plans to ensure positive outcomes Advocating for children and families within schools, agencies, and community systems Completing documentation and maintaining compliance standards Traveling within the service area to meet with families and community partners No two days are exactly alike—and every day brings an opportunity to make a meaningful difference. Requirements:
Experience counts! We accept equivalent work experience in human services (teaching, daycare, CNA, etc.) or a BA/BS degree. One year of related work = one year of college. Great with people —strong communication and empathy are a must. Experience working in human services, behavioral health, social services, education, or a related field preferred. Comfortable in a variety of human service settings. Must be able to work effectively with children experiencing Severe Emotional Disturbance (SED) and their families. Able to pass drug screen and required background checks. Must have valid driver's license.
