We are committed to redefining end of life care by setting the standard for exceptional care that prioritizes one-on-one interactions, lower caseloads, and holistic care. Join our dedicated team and make a meaningful impact in the lives of those we serve. Hospice Licensed Social Worker You will be working with other members of our Hospice Care services team to provide the very best holistic end-of-life medical, nursing, spiritual, and emotional support to patients. Essential Functions Assess the psychosocial needs of patients and families/caregivers related to terminal illness, environment, and support systems; communicate findings to the interdisciplinary team through documentation in the EMR and team collaboration. Conduct comprehensive social evaluations, including family dynamics, caregiver capacity, communication patterns, and risk factors (e.g., abuse, neglect, suicide risk); develop and implement appropriate interventions. Provide counseling and emotional support to patients and families to address stress, coping challenges, and end-of-life adjustment; deliver crisis intervention as needed. Facilitate advance care planning, including education and assistance with advance directives and durable power of attorney for healthcare, as appropriate. Assist patients and families with end-of-life planning, including education on funeral home processes, final arrangements, and what to expect at time of death. Coordinate post-death needs as appropriate, including communication with funeral homes and support for families during immediate bereavement. Provide bereavement support and education, including anticipatory and cumulative grief support. Facilitate respite care coordination, including preparation, communication, and follow-through with all involved parties. Connect patients and families with community resources to address practical and environmental needs (e.g., private duty care, Medicaid, Meals on Wheels, elder law services). Collaborate with nursing facilities and other care settings to ensure continuity and coordination of care. Participate in interdisciplinary team meetings, care planning, and discharge planning, including live discharges from hospice services. Maintain clinical competency and compliance with hospice standards and regulations. Support orientation and training of hospice personnel as needed.
Minimum Position Requirements:

Ability to work effectively in an environment that is very diverse Must possess a valid Kansas or Missouri Driver's license, dependable transportation, current vehicle insurance, and a verified safe driving record. Strong organizational and time-management skills with the ability to multitask effectively. Excellent communication and interpersonal skills. Ability to work independently and collaboratively in a fast-paced environment.
