Job Description

  • Job Summary:
  • We are seeking a Healthy Start Visitor to join the Health Department team.
This role is responsible provides family-centered, home-based services to pregnant individuals, infants, and families with young children by promoting healthy pregnancies, positive parenting practices, child development, and family self-sufficiency by conducting regular home visits, providing education, assessing family needs, connecting families with community resources, and supporting overall maternal and child health outcomes.
  • Qualifications:
  • High school diploma or equivalent required 1 year of work experience or other relevant experience Valid driver's license required or attainable within 30 days of hire Excellent communication and interpersonal skills, in both English and Spanish Ability to multitask and work in a fast-paced environment Sensitivity to diverse cultural backgrounds
  • Responsibilities:
  • Must be able to maintain HIPPA compliance Completes comprehensive family assessments and develops individualized family plans Coordinates referrals to medical providers, social service agencies, behavioral health providers, and community resources Enforces and maintains compliance with all federal, state and local laws and ordinances Follow privacy regulations and maintain confidentiality of sensitive information •
