Case Coordinator

Case Coordinators will be responsible for client advocacy, record maintenance, development of programs, behavior management to include scheduling therapeutic services, maintaining contact with the placing agency and others involved with the client. The youth to case coordinator ratio is 1:9. The Case Coordinator will be responsible for completing KDHE regulated 30-90 day case plan forms when applicable, and behavior, incident and termination summaries on all residents. In addition to program and discharge plans. The Case Coordinator will participate in any case planning initiated by the placing agency and will meet with the SSA in regards to questions of approvals in reference to case plan initiates. The Case Coordinator will supervise residents in completing placing agencies established case plan goals. The Case Coordinator also coordinates parental/home visits, provides situational counseling, as well as, providing quarterly reports and service coordination.
