Requirements of Case Manager / Family Care Coordinator Bachelors in Social Work (or other related field) Valid Driver's License and Car Insurance Benefits of Case Manager / Family Care Coordinator Flexible Work Schedule Excellent Supervised Support System Company Phone Mileage Compensation 401(k) 401(k) matching Dental insurance Employee assistance program Responsibilities of Case Manager / Family Care Coordinator Establish initial contact with families within 24 hours of referral to assess safety, Initiate safety planning, and connect families with resources to meet immediate needs; Conduct team meetings and maintaining communication with the family and providers Facilitating the implementation of each family's Action Plan Make frequent and consistent contact with DCBS personnel Provide direct services and education targeted at reducing risks, improving child well-being, and preserving child placement with the family per program guidelines.
