19 Hourly Position Type Full Time Job Shift 1st shift Education Level 4 Year Degree Travel Percentage Eligible Category Case Manager Apply Description Case management positions consist of advocating and assisting adult clients experiencing severe mental illness. Also working with children with severe emotional disturbance and their families.
ESSENTIAL FUNCTIONS
Case Manager Responsibilities:
Assist clients with accessing community resources Assess client's strengths and needs: medical, housing, education, vocation and socialization. Monitor client's progress Assist with building a network of supports Build professional relationships with community agencies. Provide crisis assistance. Assist client with advocacy efforts with other community services. Maintain appropriate contact with client. Assure client choice and access to community resources. Attend monthly supervision. Return phone calls and emails promptly. Complete and submit documentation by required deadline. Perform other related work as required and assigned. Qualifications
QUALIFICATIONS
Requires a Bachelor of Arts or Science degree in a behavioral science field, including but not limited to Psychology, Sociology, Social Work, Family Studies, Human Services, Counseling, Nursing, Behavioral Analysis, Public Health, Special Education, Gerontology, Recreational Therapy, Education, Occupational Therapy, Physical Therapy, Speech-Language Pathology, Rehabilitation Counseling, or Faith-Based Education. Candidates must have one year of post-graduate experience working with individuals with mental health, substance use disorders (SUD), or serious emotional disabilities (SED). A master's degree in one of the behavioral science disciplines listed above may be substituted for the required experience, in accordance with state regulations. Must have reliable transportation with driver's license and auto insurance as must be able to transport clients. Must possess strong written and verbal skills.
JOB REQUIREMENTS / EXPECTATIONS
Ability to sit for extended time for meetings, documentation, and clinic services. Ability to stand, walk, bend, and move within clinic and community settings. Must have car insurance and be able to transport clients for appointments and community engagement. Ability travel across regional sites when needed. Must maintain confidentiality and adhere to all regulatory and ethical standards. Performance will be evaluated by client outcomes, documentation compliance, and alignment with organizational goals.
