As a Travel Licensed Social Worker in Hyannis, MA, you will plan, organize and implement social work services to patients and families. You will assist in patient care planning for discharge and after-care to ensure continuation of treatment in an atmosphere conducive to preservation of health. Maintains contact with referring agencies and related people, and with community resources used for referral and will communicate and collaborate with healthcare teams.
