Join our team and make a meaningful difference in the lives of residents navigating aging, illness, rehabilitation, and long-term care. The Part-Time Social Worker reports directly to the Director of Social Services and plays a key role in supporting residents' psychosocial well-being. This position is responsible for completing assessments, developing individualized care plans, and providing residents and families with education, advocacy, and emotional support throughout their healthcare journey. What You'll Do Complete psychosocial assessments and develop resident-centered care plans in a timely and meaningful manner. Provide counseling, support, and education to residents and families regarding aging, illness, adjustment, coping, grief, and loss. Assist residents and families with discharge planning and access to community resources as appropriate. Demonstrate knowledge of advance directives, healthcare proxies, guardianship requirements, and related regulations. Collaborate with the interdisciplinary team to ensure residents' psychosocial needs are addressed and incorporated into the care planning process. Advocate for resident rights, dignity, and quality of life. Qualifications Massachusetts Social Worker License required ( LSW minimum ). Bachelor's Degree in Social Work from an accredited program required. MSW preferred. Minimum of 2 years of Social Services experience in a Skilled Nursing Facility required. Strong knowledge of long-term care regulations, resident rights, discharge planning, and care planning processes. Excellent communication, organizational, and interpersonal skills.
