Social Worker Affinity Healthcare - 3.6 Braintree, MA Job Details Full-time $35 - $38 an hour 1 day ago Qualifications Master's degree in social work Patient resource coordination Clinical social history assessment Community resource coordination in health services social work Master's degree Care planning in social services Full Job Description Social Worker reports to the Director of Social Services and ensure that assessments and care plans are developed in a timely and meaningful manner. Qualifications Master's degree in social work Valid and current LCSW or LMSW license Clinical social work experience working with residents with disabilities (TBI, Behavioral Health) is preferred Excellent listening and communication skills Ability to make psychosocial assessments and develop and implement care plans Ability to provide after-hours service as needed Prior experience working in Social Service department in a Skilled Nursing Facility for 2 years or more is a plus Qualified candidates must be knowledgeable regarding community resources including VNA, Elder Services, Adult Day Programs, Assisted Living Communities, Rest Homes, Senior Housing, etc. Responsibilities Provide residents and families with education and support regarding aging, illness, coping and loss. Demonstrate an understanding of advanced directives, healthcare proxies, guardianship requirements and the like. The Social Worker acts as the team leader in discharge planning and coordination of services needed for a successful transition back to the appropriate community setting. Evaluate patients (existing, new and prospective) based on their needs, limitations and desires Address patient concerns and goals while maintaining constant communication with the patient as well as a case manager Build rapport with patients and their families Coordinate care with PT and OT departments Develop care plans for patients
