This role is ideal for an RN/LPN or SW who enjoys collaboration, critical thinking, and playing a key role in driving strong patient outcomes.

What You'll Do:
Coordinate ongoing assessments of patient clinical status and level of care Evaluate and adjust care plans to ensure realistic, outcome-driven goals Collaborate with patients, families, interdisciplinary teams, and payors Manage resource utilization including services, consults, and length of stay Coordinates all aspects of discharge planning process and transitions of care Track and analyze outcomes data to improve care delivery Serve as a resource for staff on managed care and regulatory guidelines Maintain accurate, timely, and compliant documentation
What We're Looking For:
Active Massachusetts LPN, RN or Social Work license required 1+ year experience in two or more of the following: Case management Skilled nursing or sub-acute care Discharge planning Managed care / Medicare Strong understanding of managed care and reimbursement Excellent communication and critical thinking skills Ability to work independently and collaboratively At Ledgewood, we are committed to providing high-quality care in a supportive, team-oriented environment where residents and families feel valued and respected.
