Job Overview We are seeking a dedicated and compassionate Licensed Social Worker to join our dynamic team. In this vital role, you will provide comprehensive social work services to patients across various settings, including , outpatient, and community care environments. Your expertise will help improve patient outcomes by offering emotional support, case management, and resource coordination. Duties Conduct thorough patient assessments, including diagnostic evaluations and mental health screenings, to determine individual needs and develop personalized care plans. Provide clinical counseling and psychotherapy to individuals and groups, utilizing techniques such as cognitive behavioral therapy (CBT), motivational interviewing, and behavioral therapy. Assist with crisis intervention and crisis management to ensure patient safety and stability during urgent situations. Coordinate with healthcare providers to facilitate case management, discharge planning, and post-acute care arrangements. Maintain accurate medical documentation, including detailed notes on patient progress, treatment plans, and compliance with HIPAA regulations for confidentialit Extensive Proficiency in medical terminology, ICD coding (ICD-9/10), CPT coding, and electronic medical records (EMR) systems for accurate documentation. Strong knowledge of behavioral health principles including mental health counseling, diagnostic evaluation, and crisis intervention strategies. Ability to perform intake assessments effectively while Skilled in applying evidence-based practices such as applied behavior analysis (ABA), cognitive behavioral therapy (CBT), and motivational interviewing techniques. Familiarity with HIPAA compliance standards to ensure patient privacy and data security at all times. Experience working with diverse populations including PTSD.
