Our collaborative care approach incorporates Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) to minimize hospitalizations and elevate patient care standards.
Job Summary:
We are seeking a compassionate and dedicated Mental Health Provider to join our Collaborative Care team as a Care Companion/Social Worker. In this role, you will provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. Key Responsibilities:
Visit designated facilities in accordance with the schedule set with your supervisor Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns Document visit notes and maintain detailed records of interactions and interventions after each visit Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric
NP Qualifications:
Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health-related field Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings Strong communication, documentation, and relationship-building skills
