CASE COORDINATOR ICS

  • Full-Time Monday
  • Friday $21.
46 PER HOUR The Edinburg Center is seeking a Case Coordinator in our innovative, community-based Respite program to assist mental health clients living in the community in managing potential crisis situations, avoiding unnecessary hospitalization and/or transitioning successfully from the hospital to community living. ICS is a very dynamic program and provides both a residential and outreach experience!
What You Will Do:
Responsibilities include: service coordination, support, transportation, skills training, linkage and outreach as a member of an interdisciplinary team that utilizes an assertive outreach model to promote a culture in which each client's subjective experiences, points of view and preferences are recognized, respected and integrated into all treatment, rehabilitation and support services. The Case Coordinator participates in all program planning processes and provides direct services that promote client recovery, self-determination and decision-making.
Who You Are:
BA or BS preferred and at least one year of experience in the mental health field required; two years in the field preferred. First Aid, CPR, and MAP certification within three months of hire required. Skilled in the use of Microsoft Outlook within three months of hire required. Valid driver's license and reliable vehicle required.
