Job Description

Social Worker - One Care Are you interested in making a profound impact on the lives of underserved individuals and families living with complex and/ or chronic medical and behavioral conditions? Are you interested in benefits effective on the first day of employment? Are you willing to assist patients better understand their illness and coordinate their care? Do you enjoy learning of community services and resources to adopt healthy behaviors by conducting the key functions listed in the duties and responsibilities.? Are you interested in a loan repayment program and funding for license renewals? Can you commit to work on a Primary Care Team Model as well as with clients independently to provide appropriate follow-up?
In this role you will:
Be responsible for addressing the social service needs of One Care members. Provide follow-up sessions as needed with One Care health center clients or coordinating services by phone. Provide brief counseling, advocacy, & resource referrals as needed. Participate in case conference as needed. Conduct home visits alone or in conjunction with member(s) of the interdisciplinary team as appropriate, either at the patient s home or in other locations in the community. Be an active & integral member of the primary care/BH team. Advocate for patients with outside agencies such as social service organizations, schools, etc. Provide crisis intervention as needed.
The successful applicant will offer:
Human services experience desired. Ability to work with children & adults, & the elderly. Minimum of a bachelor s degree in the human service field (e.g., psychology, sociology) required, BSW preferred; bilingual/bicultural candidates strongly desired. LSW required History of good attendance & positive work attitude.
