Social Worker - One Care Are you interested in making a profound impact on the lives of underserved individuals and families living with complex and/ or chronic medical and behavioral conditions? Can you commit to work on a Primary Care Team Model as well as with clients independently to provide appropriate follow-up?

In this role you will:

Be responsible for addressing the social service needs of One Care members. Provide follow-up sessions as needed with One Care health center clients or coordinating services by phone. Provide brief counseling, advocacy, & resource referrals as needed. Participate in case conference as needed. Conduct home visits alone or in conjunction with member(s) of the interdisciplinary team as appropriate, either at the patient s home or in other locations in the community. Be an active & integral member of the primary care/BH team. Advocate for patients with outside agencies such as social service organizations, schools, etc. Provide crisis intervention as needed.

The successful applicant will offer:

Human services experience desired. Ability to work with children & adults, & the elderly. Minimum of a bachelor s degree in the human service field (e.g., psychology, sociology) required, BSW preferred; bilingual/bicultural candidates strongly desired. LSW required History of good attendance & positive work attitude.
