Complete the MDS for resident's in a timely fashion Develop and implement individualized care plans in collaboration with the healthcare team. Facilitate communication between residents, families, and staff to enhance care. Provide counseling and support to residents and their families during transitions. Organize and lead group activities to promote social interaction and engagement. Advocate for residents' rights and ensure their needs are met within the facility.
