Social Worker Per-Diem up to 16 hours per week Vantage at New Bedford New Bedford, MA Job Details Per diem 1 day ago Qualifications Collaborate with healthcare professionals Direct patient care Resident's rights Patient assessment Social work Patient advocacy Family counseling Mid-level MDS Care plan development Client advocacy in social work Patient interaction Full Job Description As a per-diem social worker, your responsibilities are to: provide essential support and care for residents in a nursing home, consider the following responsibilities for a per-diem social worker, up to 16 hours per week: Conduct assessments to identify residents' emotional and social needs. Complete the MDS for resident's in a timely fashion Develop and implement individualized care plans in collaboration with the healthcare team. Facilitate communication between residents, families, and staff to enhance care. Provide counseling and support to residents and their families during transitions. Organize and lead group activities to promote social interaction and engagement. Advocate for residents' rights and ensure their needs are met within the facility.
