reviews Full-time The Hockomock Area YMCA is hiring a Pathways Forward Manager to lead intake, navigation, and follow-up for the Pathways Forward Program, ensuring individuals and families are connected to the resources and support needed to achieve stability and long-term well-being. This role serves as the primary point of contact for individuals and families seeking support beyond traditional scholarship through the YMCA's Social Action Services (SAS) Center. Working within a wraparound service model, the Pathways Forward Manager oversees intake, screening, assessment, service planning, referrals, and ongoing follow-up to ensure participants are connected to appropriate supports across financial assistance, food access, mental health, childcare, education, and chronic disease programs. This role is highly collaborative and works closely with internal YMCA departments and external community partners to provide coordinated, responsive, and person-centered support that strengthens individual and family stability, builds capacity, and promotes long-term well-being. Essential Functions Serve as the primary point of contact for Pathways Forward participants Conduct intake conversations and screenings to identify needs and appropriate supports Build supportive relationships and provide a welcoming participant experience Maintain accurate participant documentation in Casebook and related systems Develop individualized support plans and coordinate referrals to YMCA programs and community partners Facilitate warm handoffs and ongoing participant follow-up Monitor participant progress, address barriers to engagement, and adjust support plans as needed Engage the Social Worker or leadership team when additional support or escalation is required Support participants through assessment, service connection, monitoring, and stabilization Collaborate with YMCA teams, including Scholarship, Food Access, Childcare, Chronic Disease, and Social Action staff Participate in case review meetings and support a coordinated, team-based approach to service delivery Track intake, referrals, service connections, and follow-up outcomes Support grant reporting and program documentation requirements Coordinate with Oasis and other systems for service tracking as needed Bachelor's degree in human services, social work, psychology, public health, business, or a related field preferred 2-4 years of experience in case management, community outreach, social services, or related support work preferred Strong interpersonal, communication, and relationship-building skills Ability to manage multiple participants, priorities, and follow-up responsibilities in a fast-paced environment Experience working with diverse populations and community-based resources Strong organizational skills with the ability to maintain accurate documentation and records Proficiency in Microsoft Office and familiarity with case management systems such as Casebook preferred
