Develop a comprehensive social history. Providing social intervention, such as counseling, as patient advocate, as liaison with the community, or regarding financial or emotional issues. Maintaining social service progress notes on patient medical records which reflect any changes in patient psycho-social or physical status, evaluation of family interactions, identification of new problem areas, and review of social plans, goals and progress made. Develop initial discharge plan and document in clinical record, revising until discharge with available community services.
Qualifications:
EDUCATION
B.A. or M.A. degree in social work or related field. Must be a licensed social worker at the LSW level of licensure or higher.
TRAINING AND EXPERIENCE
At least one year direct social work experience in a long term care facility. Must be a licensed social worker at the LSW level of licensure or higher. Must have a genuine interest in working with geriatric patients and a sincere concern for their welfare and happiness.
