$34.42 - $44.00 / hour, depending on years of experience Centurion is proud to be the provider of comprehensive services to the Maryland Department of Public Safety and Correctional Services. We are currently seeking a Full-time Discharge Planner (Social Worker) to join our re-entry team at Baltimore Central Booking and Intake Center (BCBIC) located in Baltimore, Maryland. The Discharge Planner (Social Worker - Re-Entry) completes discharge planning for patients referred to the multidisciplinary team based on the determination that community resources will be required at the time of release. This person will ensure that discharge planning is completed within the time period required by the client for the type of referral. They will accomplish timely and appropriate discharge plans through assessment of the patient's post-release needs and the provision of services necessary to meet those needs. This person will act as a liaison between correctional healthcare providers and community agencies. They will collaborate and coordinate with all applicable parties (social work, security, etc) for release planning. This person will maintain accurate and timely records related to release planning, including active files of available community resources to facilitate the release planning. Qualifications Bachelor's Degree in Psychology, Social Work, Counseling, or related field from an accredited educational institution; Master's Degree preferred Minimum of two years' experience in release planning preferred Prior experience in working in a correctional healthcare program, criminal justice, or forensics preferred Must maintain current tuberculosis documentation and active CPR certification (BLS) Ability to obtain a security clearance, to include drug screen and criminal background check We offer excellent compensation and comprehensive benefits for our full-time team members including: Health, dental, vision, disability and life insurance 401(k) with company match Generous paid time off Paid holidays Flexible Spending Account Continuing Education benefits Much more...
