Summary Provides clinical social work services to assigned patient care areas, supporting patients, families, and care teams in addressing psychosocial needs and barriers to care. Essential Functions Develops and implements plans to help resolve or reduce issues affecting patient care, in collaboration with patients, families, physicians, staff, and community resources. Provides patient education, crisis intervention, short-term counseling, discharge planning, and referrals to community and government agencies. Documents assessments, interventions, care plans, and follow-up in the medical record in a timely and accurate manner. Uses available resources and sound judgment to support positive outcomes for patients, visitors, and staff. Identifies opportunities to improve departmental systems and processes and contributes ideas for solutions. Responds to urgent and unplanned situations by assessing needs, taking appropriate action, and notifying management as needed. Prepares incident/occurrence reports when necessary and communicates findings to appropriate leadership. Qualifications Master's degree in Social Work required Current Maryland social work licensure or license eligibility at the LGSW, LCSW, or LCSW-C level
Salary Range:
Minimum 32.40/hour
  • Maximum 53.
48/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
