Job Description

Reporting to the Program Manager, the Case Coordinator, Homelessness Prevention provides supportive case management primarily focused on helping individuals/households obtain and maintain safe, stable housing. This role includes assessing housing needs and barriers, creating housing stability plans, connecting clients to community resources, and providing ongoing coaching (financial literacy/budgeting). Responsibilities also include coordinating with treatment teams/providers, ensuring follow-through appointments, conducting monthly housing inspections for safety/cleanliness and transporting clients as needed. The coordinator completes intake paperwork, maintains releases of information, conducts assessments and annual reviews, documents all case management activities, and ensures client files and recertification documentation are complete. Minimum Bachelor's Degree in social work or related field. Minimum 2 years experience providing direct support to disabled and/or homeless populations. Valid driver's license required. Knowledge of Microsoft Office and PC-based systems. Excellent written and verbal communication skills. Strong organizational skills with attention to detail and follow-through. Ability to work independently, take initiative, and manage multiple priorities. Demonstrated ability to relate well to individuals of diverse races, ethnicities, genders, sexual orientations, socioeconomic backgrounds, and religions All positions at Humanim are to encourage supported employment for those people we serve who have expressed an interest in such employment, including consumers with long-term mental illness. To that end, every employee will promptly refer any consumer with an interest in obtaining supported employment to appropriate Vocational Staff and will actively participate as needed in the Evidence-Based Practice in supported employment initiative/model. All Humanim staff must be able to relate well to clients, board members, and staff of different races, ethnicities, genders, sexual orientations, socio-economic backgrounds, and religions.
