We are seeking a compassionate and dedicated Social Worker/Care Companion to join our Collaborative Care team at Carease Health - MD. As a valued member of our team, you will play a vital role in providing emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. About the
Role:
As a Social Worker/Care Companion, you will visit designated facilities in accordance with the schedule set with your supervisor, conduct assessments of residents to identify emotional, cognitive, and psychosocial needs, and implement short-term, solution-focused therapy and supportive interventions to address immediate concerns. You will document visit notes and maintain detailed records of interactions and interventions after each visit, and collaborate with psychiatric professionals to ensure comprehensive care.
Key Responsibilities:
  • Visit designated facilities in accordance with the schedule set with your supervisor
  • Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
  • Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns
  • Document visit notes and maintain detailed records of interactions and interventions after each visit
  • Collaborate with psychiatric professionals to ensure comprehensive care
Qualifications:
  • Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health-related field
  • Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
  • Strong communication, documentation, and relationship-building skills
Education:
Associate Degree

    • Pick the day and hours that work for you! Approximately 12 hours per week during Monday-Friday, within regular 8-5 hours
